Matching a pattern only when in current topic
 
 

This is probably an obvious question, but is there a way to restrict matching to only when in the current topic? Possibly through a rejoinder somehow?

I have students who ask

Any prior history of headaches? - No
Any history of back pain? - no
Etc.

I can write patterns like ?: (<< [history prior ever before] headache >>) which works, but sometimes they ask

Any prior history of headaches? - No
How about back pain? - no

So I would like to write some very general patterns in this topic like ?: (headache) but only have those match if we are already in this topic and not match if we get to this topic from some other topic.
